For your first visit, please bring:
- Any X-rays from a previous dentist (only if you have them, if not, your previous dentist can also forward X-rays to our office if you ask)
- A valid form of ID
- A list of medications you are currently taking
- A list of any health concerns, ongoing issues, and past surgeries
- Your dental insurance card (if applicable)
